Frozen meals, prepared locally, are provided to many area agencies and organizations for distribution to their clients, parishioners, patients, and residents. Contact staff at Five Rivers Health Centers if you are a patient who is suffering from food insecurity and would benefit from receiving these meals when you come in for an appointment.

Joint office of citizen complaints. Investigates complaints about government agencies and services for residents of Montgomery County. Also investigates complaints about care provided in nursing homes, adult care facilities, group homes and in-home care services.

Access many resources and services through CSSMV including family stabilization services, refugee resettlement, behavioral health support, guardianship services and services for older adults.

Artemis Center empowers survivors of domestic violence to make decisions and choices that will establish and maintain safety for themselves and their children. By working collaboratively with other community services, Artemis Center advocates are able to coordinate needed services. Additionally, community education and advocacy programs help to create community best practice responses for survivors of domestic violence.

Five Rivers Health Centers will assist anyone in the community with insurance applications and Medicaid/Medicare enrollment. You do not need to be a patient here to receive this free assistance. Please call and speak with our Outreach & Enrollment team to get more information and assistance.

Depending on the patient needs, most insurance providers offer some type of transportation assistance to and from an appointment at Five Rivers Health Centers. We also offer Uber Health transportation services for many of our patients. Ask our staff members about these free transportation options.

Speak with the women's health staff at Five Rivers Health Centers to be measured for a properly-fitting bra (maternity or nursing). This free service is available to any of our patients who have challenges finding or affording specialty sizes and types of bras.

With a team of around 130 active volunteers, the Brunner Literacy Center offers educational programs in four main areas. Students are matched with the program area that is best aligned with their goals and starting skill levels, and each student's individualized plan is further customized according to desired timeline, availability, learning styles, and more.

As a result, no father is listed on the child’s birth certificate. This program helps with establishing paternity so that the child has a legal father listed on his/her birth certificate.

Five Rivers Healthy Start supplies diapers to our patients from the Montgomery County Diaper Bank.  Five Rivers Health Centers patients & partnering agencies on the Community Action Network (DMC Public Health & Catholic Social Services) have access to provide diapers to clients in need.  If you are a Five Rivers Health Center patient, for more info, call 937-281-6592. For Non-Five Rivers patients, you may be able to access diapers from the Diaper Bank via one of our partnering agencies.
