To reach the provider on-call after hours, follow the prompts on the after hours message (937) 461-1376.
Please restrict those calls to URGENT medical matters only. The provider on-call will NOT return calls to change appointments or refills).
If you will be more than 15 minutes late to your appointment, please call the center to verify that you can still be seen. If you cannot keep your appointment, please call at least 24 hours in advance in order to open up that time for other patients.
