We will update this website with new patient appointment information when it is available.
The following changes have been made based on the current changes in CDC guidelines and Governor DeWine’s guidance on masking and other protocols for fully vaccinated individuals. The updated CDC guidelines excluded healthcare facilities and recommend continued use of protective gear at all times during patient care for all healthcare staff regardless of vaccination status.
Masks will still be required for all staff, patients and visitors (vaccinated or not vaccinated) in all Five Rivers public areas and during patient care.
Public areas include waiting rooms, hallways in patient care areas, exam rooms, pharmacy front desk/window and all nursing stations during patient care.
We require all patients entering our health centers to properly wear face coverings. Staff will be wearing face shields and other PPE when interacting with patients. If you are experiencing COVID symptoms or are not able to see your provider in-person, we provide tele-health appointments. Please contact our offices with any questions or concerns.

Patients Receiving Quality Care
"Your staff is wonderful, I always feel comfortable during my visits, thank you all so very much."
"It was easier to do this visit over the phone than it is for me to get there in person. Dr Hall listened to my concerns and was very attentive to my concerns and is genuinely interested in my wellbeing. She is actually one of the only Dr’s. that has actually cared and I appreciate that more than words can say. Finally, I feel good about myself."
Five Rivers Health Centers CEO, Gina McFarlane-El, has once again been recognized as one of our region's most influential women.
